Near-Miss: The Utility Marks Had Faded — Then the Backhoe Found the Gas Main

A crew was trenching to install drain pipe when a backhoe struck a 3-inch gas main. According to an OSHA accident report, the job had been marked through the utility-locate process days earlier, but the markings had faded.
A worker standing near the backhoe was hit in the face and chest by flying dirt and debris when the line was struck. He was taken to the hospital and later released.
That outcome could easily have been worse.
Why this belongs in the near-miss file
Crews see faded paint all the time. Traffic, rain, dust, grading, tires, and normal jobsite activity can turn clear locate marks into guesses. The dangerous part is how quickly a crew can get used to working around marks that are “probably still right.”
The correct response to a questionable locate is not to keep digging carefully and hope the original marks are close enough. It is to stop, protect the area, refresh the locate information, and use safe means to verify the utility as the work approaches it.
OSHA's excavation standard requires employers to determine the estimated location of underground installations before excavation and, as operations approach those installations, determine their exact location by safe and acceptable means.
A simple field rule
If the marks are no longer good enough for a new employee walking onto the site to understand them, they are not good enough to excavate from.
Before the bucket, trencher, plow, or drill moves into the tolerance zone:
Stop if paint or flags are missing, disturbed, or unclear.
Re-notify the locate service when required and get the area remarked.
Use vacuum excavation or another accepted soft-dig method to expose critical utilities.
Keep people out of the line of fire around pressurized facilities.
Rebrief the crew whenever conditions or markings change.
A near-miss is useful only if somebody changes the next decision because of it.
